Adding A Form To Blogger

Does anyone know how I can add a response form to my blogger blog using a gadget?

I'd really like to introduce a new element to my blog but I cannot find any tutorials about how to do it.

What do you mean with a response form? Is it like a guest book or shoutbox? I think Blogger enable us to add gadgets using third party's code. So if your element (whatever it is) is coded properly, it can added on Blogger sidebar....I guess.
